CDAA Services
CDAA will help you manage all the care and services you need. You will work closely with a specially trained Care Manager who helps you better understand and manage your conditions. Your Care Manager collaborates with you to create a plan of care that meets all your physical, mental health and social service needs and find the services and programs that are right for you, your child, and your family.
Your Care Manager Will:
Improve the quality of your health and social life.
Assist with Medicaid re-certification & applying for SNAP & SSI/SSDI benefits.
Assist with housing applications, legal resources, transportation, or other issues.
Assistance with free government cell phone.
Assist with utility bills.
Assist with immigration resources.
Make appointments with your doctor or specialist for you.
Work with your providers to make sure you get the care you need.
Help you understand your chronic health conditions and give you information to help manage problems like diabetes, asthma, or high blood pressure.
